It is a document used by the Federal Bureau of Investigation (FBI) to provide an Identity history Summary Check for an individual based on the stored fingerprints. 

The FBI card is also commonly referred to as the FD-258/FD-1164 card. While you are in the USA, you can apply for the FBI check by providing digital fingerprints through authorized locations. However, if you are located outside the USA, you would need to complete the fingerprints on the FBI card as there is no alternative way to submit the fingerprints to the FBI. Print the FBI card on sturdy cardstock paper and approach a police station or local fingerprinting agency to complete your fingerprints. These can be captured with conventional ink & roll methods or advanced LiveScan devices on the FBI card. It is crucial to note here that LiveScan Digital Fingerprinting captures the digital images of your fingerprints. Note that details on the card like Namee, Date of Birth, Citizenship, etc must also be filled out.
